MDS Coordinator

Full-time

The primary purpose of this position is to coordinate the RAI process assuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.

Responsibilities:

  • Monitors and guides the completion of assessments in a timely manner

  • Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of validation

  • Schedules and facilitates care plan conferences

  • Facilitates monthly QM meetings

  • Manages Care Planning Process

Qualifications:

  • Must hold a Nursing Degree from an accredited college or university

  • A current, unencumbered, active RN license and CPR

  • At least 2 years experience in Long Term Care as an MDS Coordinator

Skills:

  • Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid

  • Excellent written and verbal communication skills and critical thinking abilities

  • Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility

  • Ability to organize and prioritize

Maintenance Assistant

Full-time

The ideal candidate must be devoted and compassionate about their career.

Responsibilities:

  • Ability to understand the policies and procedures of the department

  • Basic plumbing, electrical, and drywall skills. 

  • Must be very handy and able to make repairs.

  • Assist the environmental manager with regulations and upkeep of the state regulations

  • Knowledge on how to do minor repairs in the building

  • Ability to make independent decisions

  • Maintain care and use of the supplies and equipment

  • May be required to lift, push, pull, and move equipment

  • Attend staff meeting as directed

Qualifications:

  • High school diploma or GED preferred

  • 1 year of maintenance experience is required

Dietary Aides

Part-time

Responsibilities:

  • Janitorial duties

  • Taking out trash

  • Keeping kitchen clean, carts, etc.

  • Delivering food and beverages to units

Qualifications:

  • High school diploma or GED

  • Dietary/Kitchen experience preferred
